Care Coordinator

Developed by Optum Technology, Care Coordinator was primarily used by Mayo Clinics. The application was designed to reduce the likelihood of complex care patients returning to the hospital or emergency room. Through shadowing and interviewing care coordinators at Mayo clinics across the country, I gained a better understanding of their challenges and how they work with patients. My job then was to help design the user experience for this application in the Salesforce Lightning platform in collaboration with the product team.
